Last name, first name OR Organization.  "Title of survey."  Survey. Day Month Year.

Example (Organization):

Encyclopedia Article:

Author's last name, first name. "Title of the article." Name of the encyclopedia. Date.
Example:  Barr, William. "Northwest Passage." World Book Encyclopedia. 1998 ed.


Magazine Article:
Author's last name, first name. "Title of the article." Name of the magazine. Date on magazine: Pages.
Example:  Safer, Barbara. "Secret Messages." Cricket Sept. 1998: 29-33.


Newspaper Article:
Author's last, first name. "Title of the article." Name of the newspaper. Date(day month year), Section: page.
Example:  Salemy, Shirley. "Plan Will Aid Children." DesMoines Register. 2 Sept 1998, sec. M: 1.


CD/DVD:
Title of the CD. CD-ROM. Place where published: Publishing company, Date.
Example:  Grolier Multimedia Encyclopedia. CD-ROM. Danbury, CT: Grolier Interactive, 1998.


TV Show:
"Title of the Program." Name of the show. Network. Local Channel, Place, Date (day month year).
Example:  "Crime and Punishment." Nightline in Primetime. ABC. WKOW, Madison, WI. 3 Sept 1998.


E-mail:
Person or organization who sent the e-mail. Their e-mail address. "Subject of the e-mail." Date sent. E-mail to the author. Date read.
Example:  Write Source.
Email Address.  "Citing Internet Sources." 4 Sept. 1998. E-mail to the author. 8 Sept. 1998.
